Most bone tumors are osteolytic. The most reliable indicator in determining whether these lesions are benign or malignant is the zone of transition between the lesion and the adjacent normal bone .
Once we have decided whether a bone lesion is sclerotic or osteolytic and whether it has a well-defined or ill-defined margins, the next question should be: how old is the patient? Age is the most important clinical clue. Finally other clues need to be considered, such as a lesion localization within the skeleton and within the bone, any periosteal reaction, cortical destruction, matrix calcifications, etc.
