Diffirentiating the Causes of an Opacified Hemithorax
Causes:
1-Atelectasis of entire lung.
-Loss of volume of the affected lung.
-Visceral and parietal pleura do not separate from each other.
-There is shift of the heart and trachea toward side of opacification(toward side of loss of volume).
2-A large pleural effusion.
-If an effusion(whatever the fluid is)fills the entire hemithorax.
-It acts like a mass.
-Pushing the heart and trachea away from side of opacification.
3-Pneumonia of an entire lung.
-The hemithorax is opaque and there is no shift of the heart and trachea.
-There may be an air bronchogram sign present.
4-Post pneumonectomy
-When the entire lung is removed,there is volume loss on the pneumonectomized side resulting in shift of trachea and heart toward affected side.
-The hemithorax eventually fibroses and become opaque.
-Clues:There is frequently a resected fifth rib and/or surgical clips.
Important Notes:
-In atelectasis,there is a shift toward the side of opacification.
-In pleural effusion,there is a shift away from the side of opacification.
-In pneumonia,there is no shift.
-In pneumonectomy,there is shift toward the same side of opacification with evidence of surgical clips and/or resection of fifth rib.
